Below are providers that offer COVID-19 Vaccinations. You can use the links below for scheduling. Please also note that Teller County Public Health is not responsible for scheduling and appointment availability for locations other than Public Health clinics.

FYI regarding Johnson & Johnson vaccine: Women between the ages of 18-49 are considered at a higher risk of developing complications (7 persons per 1 million doses) and should review the emergency use authorization carefully before scheduling a Johnson & Johnson vaccine appointment

Peak Vista Community Health Centers is administering the Moderna COVID-19 Vaccine at its Health Center at Divide, 34 Hybrook Road S, Divide, CO on Fridays. Appointments are required. People who want to get the vaccine should call 719-344-7714 to schedule an appointment

You can request an immunization record for yourself or your child? Immunization records from CIIS may not be complete; the records include what has been reported to and entered into CIIS.

There are several ways to request a copy of your immunization record through CDPHE: the public portal and the Request to Release Immunization Record form. You can also request a copy of your immunization record from your provider. 

Patients and guardians of minors can now use the Colorado Immunization Information System (CIIS) Public Portal to view and print an official record of their immunizations.
